Consumer Access to Health Records and Complaint Instructions (Texas HB 4224 Compliance)
Effective September 1, 2025 — Consumer Rights Notice
In accordance with Texas Health & Safety Code Section 181.105 (House Bill 4224) and applicable state law, we provide the following information to assist consumers in accessing their health care records, contacting relevant regulatory authorities, and filing complaints.
1) Request Your Health Care Records
You have the right to request a copy of your health care records. To make a request, submit a written request using one of the following methods:

In-person: Visit our front desk during regular business hours and request a Release of Information form.
Please, include with your request:
- Your full legal name
- Date of birth
- Dates of service
- Description of the records you are requesting
- Preferred delivery method (secure email, mail, etc.)
- Your signature (or signature of legal representative with authorization)
We will process your request in accordance with Texas law and HIPAA. Reasonable fees for copies may apply in accordance with Texas law.
2) Contact the Licensing or Disciplinary Authority
If you need to contact the regulatory authority that licenses or disciplines providers or facilities like ours, you may use the appropriate agency below based on provider type:

3) File a Consumer Complaint
If you believe your rights regarding access to records or other patient protections have been violated, you may file a complaint:
